House of Paws requires all customers to bring in the appropriate forms below corresponding to your service. (Daycare and/or Boarding)
These forms are required within one week of booking reservations for boarding and/or daycare. Also, all House of Paws guests need to come wearing a collar with a leash attached.
Please fill out these forms in their entirety for each pet that will be staying with us.
All House of Paws canines are required to be up to date on Rabies, Bordetella and DHLPP vaccines. Vaccination records are required within one week of booking reservations for any of our services.
We require Bordetella vaccinations every 6 months.
All House of Paws felines are required to be up to date on Rabies and FVRCP vaccines.
